ElderLynk care coordination services assists older adults who are having difficulty living independently in their homes by providing appropriate community-based linkages to services that can help. A comprehensive assessment is conducted to determine the individual's needs and expectations, and a plan of care is established. Reassessments and monitoring is done on a regular basis to note changes in the client's condition, circumstances, and needs.
