A long-term care facility providing skilled nursing care and adult day care services. We can transport adults here free of charge. We also provide limited community transportation at no charge for local doctor appointments, grocery store & pharmacy trips at no cost. Call 660-332-4315 and ask for Susie or Penny.

Advocates for the rights of all individuals with developmental disabilities. Serve as an agent for change in programs, systems, and legislation. Promote community education/awareness and family support regarding developmental disabilities. 4 Core Services are Peer Support, Information and Referral, Independent Living Skills Training, and Advocacy. Other services include, but are not limited to, Accessibility Services, Circuit breaker services, Assistive Technology including TAP Telephones, Equipment Loan Program, Transitioning, Alternative Format Services,Consumer Directed Services (CDS), and Medicaid Transportation.

Eligibility Criteria: Must be 60 years +, no income guidelines, as of July 1 we will no longer provide the older worker job training program. Residency required in counties listed below, can refer to other agencies if outside district.

Description of Services and Other Information:
Nutrition Services (Generic special diets available):
1. Congregate meals served at 17 Senior Centers at least 5 days weekly
2. Home delivered meals 5 days per week extra meals available for evenings and weekends (depending on need)
In-Home Services help senior remain in home through personal care, Respite and homemaker / chore services
Transportation: Can provide transportation to doctors and shopping through OATS. The agency has limited resources for Client Reimbursed Transportation covering non-emergency medical trips only.
Legal Services: Resolution of non-criminal legal matters and protection of legal rights
Ombudsman Program: Trained volunteers advocating for qualtiy care for long-term care residents and many other programs available.
Provides information and assistance services to caregivers, family members and seniors. Gives access to state and local organizations serving the elderly and disabled.

Transportation: For age 60+ can provide transportation to doctors and shopping through OATS, Provide A Ride volunteer transportation, medical only, client reimbursed transportation, no family driver.
Legal Services: free resolution of non-criminal legal matters and protection of legal rights
Long Term Care Ombudsman Program- Advocates on behalf of nursing home, residential care residents,
and many other programs available including health screenings, outreach, silver haired legislature, public education and information.
Enhanced information and assistance which assists seniors and caregivers with one on one service.
