Description of Services and Other Information:
Include, but are not limited to:
*Screenings—Diabetic, cholesterol, blood pressure, hemoglobin, AIDS, pregnancy, etc.*Prenatal Program—Prenatal and high risk pregnancy case management. The client is responsible for paying for hospital delivery services and delivery physician. They can issue temporary Medicaid cards to pregnant mothers. *Family Planning-Birth control, pregnancy testing, pap smears, etc. *Wic *Healthy Children and Youth Examinations by appointment only *Home Health Services and Community Nursing -- visits made upon physician’s referral. Skilled nursing Care provided including health assessments, teaching, dressing changes, colostomy care, and IV therapy.
