Advancing Chronic Care through Excellence in Systems and Support (ACCESS)
Program provides technical assistance to area healthcare clinics to develop a patient-centered approach in the delivery of chronic disease care and self-management classes offered in clinics. Currently, NMRHN staff members are working with Complete Family Medicine and Kirksville Family Medicine/Residency Clinic to implement systems change. Self-management classes are offered to anyone in the area in need of training. Contact Sarah Eber, MPH, RD, LD, CDE, ACCESS Chronic Disease Management Consultant at 660-665-0330 to learn more about when the classes are offered or if your clinic would like to participate in the project.
NMRHN is also partnering with the Northeast Missouri Regional Arthritis Center to offer Stanford University's Chronic Disease Self-Management Program training in community settings, as well as leader trainings, throughout the Network's 11- county service area.
