Adair County Health Department

Children youth/sport physical clinics at health department and one area school, family planning/STD clinics, and lead testing clinics for children.

Clark County Health Department (2006 & 2007)

Offer stroke and hypertension prevention program as well as blood sugar screenings two days per week, each patient will receive counseling on risk factors.

Knox County Health Department

Provide at least three clinics per year for lipid and blood sugar testing, assessment of risk factors for cardiac disease and diabetes, and blood pressure/heart rate screening; and provide patient referrals and appropriate health education information when appropriate.

Lewis County Health Department (2006 & 2007)

Offer free men’s wellness screenings.

Linn County Health Department

Increase the number of medically disabled people receiving appropriate foot care for conditions such as diabetes, peripheral vascular disease, blindness/severe visual impairment, and severe disabling arthritis.

Macon County Health Department

Provide blood lead testing to uninsured/underinsured children 6-72 months of age.  Revised Missouri Statutes (Sections 701.326-701.349) requires any child under the age of six years living in or visiting the Universal Testing (high-risk) area for 10 hours per week or more be tested annually for lead.  Macon County is considered a high-risk area.  Testing, follow-up, and education will be coordinated through WIC, immunization clinics, other client encounters and during annual Head Start screenings.

Mark Twain Area Counseling Center

Provide mental health assessments, case management (including in-home visits) and referrals to individuals & families who have no other payment method.

North Central Missouri Mental Health Center

Provide mental health assessments, case management (including in-home visits), and referrals to individuals & families who have no other payment method.

Northeast Missouri Health Council

Facilitate enrollment in medical aid programs; and aAssist clients in a) navigating the rural health/social service system, b) gaining access to comprehensive care & patient education, and c) facilitating positive health care choices through clinic visits.  Additionally, free mental health screenings will be offered at three clinics during National Depression Week, free alcohol screenings will be held during Alcohol Awareness Month, free dental screenings will be scheduled in Milan (to target the Hispanic population), and free screenings/educational programs will be provided on childhood obesity.                         

Northeast Regional Medical Center

Provide mammography (15), bone density (15) and prostate (20) diagnostics for low-income, uninsured/underinsured persons through a referral and voucher process.

 

Preferred Family Healthcare

Provide individual adult outpatient mental health, family/marital counseling sessions, and initial screening consultations to consumers who are not currently receiving services due to low income or no insurance.

 

Putnam County Health Department

Provide on-site clinics in local communities to monitor blood pressures, heart rates and respirations (with at least two clinics targeted to the Amish population).

Putnam County Memorial Hospital

Purchase Cholestech LDX and GDX systems for providing four mobile screening clinics for blood pressure, diabetes, cholesterol, and anemia each year.

Samaritan Memorial Hospital

Provide well women clinics every other month.

Schuyler County Health Department

Purchase Cholestech LDX system for screenings, provide lipid and glucose screenings, and offer at least two on-site blood pressure screenings per year (screenings would be targeted toward the Amish population).

Scotland County Health Department

Purchase Cholestech LDX system for screenings, provide lipid and glucose screenings, and offer at least two on-site blood pressure screenings per year (screenings would be targeted toward the Mennonite population).

Scotland County Memorial Hospital

Provide a variety of free screenings monthly

Shelby County Health Department (2006 & 2007)

Offer free men’s wellness screenings.

 

Sullivan County Health Department

Provide in-home postpartum visits and deliver new baby packets, primarily to the Hispanic population within Sullivan County.

Sullivan County Memorial Hospital

Expand clinic services at two locations by adding four physician hours per week, and offer free blood pressure and blood sugar screenings at clinics for those meeting the charity needs criteria.
